Instructions
Main visa application form, filled out by ALL non-immigrant visa applicants (must be completed online using Roman characters)*
DS-157Supplementary visa form, filled out by MALE visa applicants AGED 16-45 applying for non-immigrant visas of ALL classes. (exclusions: diplomatic visas of classes A (A1, A2, A3) and  G (G1, G2, G3, G4), and NATO visas)
DS-158Supplementary visa form, filled out by visa applicants applying for STUDENT (F1, M1) and EXCHANGE VISITOR (J1) visas, and by their DEPENDENTS (F2, M2, J2)

* You can fill out an electronic visa application form DS-156 yourself using our instructions either by accessing it from your home or work computer or by visiting an Internet café, a post office or library that has Internet access in your hometown, even if you do not speak English.  We recommend that you read the instructions carefully before completing the form.  If you are not familiar with a PC or you have never surfed the Internet, you can go to one of the following Internet cafés in Minsk where you can receive assistance filling out your DS-156.
